Healthy Respect’s PTKs

Are for young people ages 13+ and those 19-25 with additional vulnerabilities who may need enhanced support to access services.

• They must be completed and sent to lab with the support of a worker.
• Please remember to record this PTK reference number on Healthy Respect service recording tick sheet.
• Record anonymously, date of test, PTK reference number and note to follow up
